What to Expect
First Visit
Most Emory Executive Physicals will be completed in one half-day. For your convenience, we offer most tests within the same building. All components of the exam are coordinated and managed by the Executive Health staff. Prior to your visit please print, complete and fax a copy of the pre-scheduling worksheet. A pre-scheduling worksheet is available for corporate accounts and self-pay individuals.
What should I bring to my first appointment?
If you haven't already provided information, please bring:
- Completed Health History Questionnaire (provided after scheduling)
- A copy of your medical records. If possible, please try to send any applicable medical records one to two weeks prior to your appointment so our physicians can become familiar with your medical situation.

- A detailed itinerary for your Emory Executive Physical, which will include any additional items you may need to bring to your appointment, a campus map and a Health History Questionnaire will be mailed to you.
