When calling to schedule your first visit, please have the following information available:

  • Patient's Name, Address, and Telephone Number
  • Date of Birth
  • Social Security Number
  • Diagnosis
  • Name, Address, and Telephone Number of Treating Physician
  • Insurance Information

Prepare for Your Visit

Medical records may be requested for review prior to scheduling an appointment to ensure each patient diagnosis is scheduled with the appropriate physician.

Before your first appointment with Emory Neurological Surgery, please do the following:

  • Complete the medical history form. (Download or request forms below.)
  • Obtain medical records, slides and films from scans from your other health care providers.
  • Gather any notes on your condition.

Please bring all of these materials with you. You must bring in films/slides from scans to your first visit unless specifically indicated by our office.

These steps will help us expedite your appointment and assure we have the appropriate information needed to provide the best advice possible during your initial consultation.

You can then print the form, fill in all information, and either send it by mail to the office of the physician with whom you have an appointment or bring the completed form with you to your appointment. Directions and Parking

The Department of Neurological Surgery uses Emory University Hospital and Children's Healthcare of Atlanta at Egleston for our neurosurgical in-hospital care.  In addition, we have partnerships with Grady Memorial Hospital and Emory Crawford Long Hospital, where Emory physicians also provide neurosurgical care.

Directions and Parking

Emergency neurological care, critical/intensive care, and surgical and interventional neuroradiological procedures are performed at Emory University Hospital on the campus grounds.
